PRIVATE INSURANCE

We will bill insurance companies that require direct submission from our office. However, due to the increasing number of insurance companies, we are unable to directly bill them all. If your insurance company is one we do not bill directly, we will gladly provide the proper items for you to bill them.

Please bring your insurance card(s) with you to your appointment.

If you have a managed care insurance plan, be sure to obtain the proper referral and authorization from your primary care physician. Prior arrangements for authorization and referral will ensure that you will not be personally responsible for the charges.
MEDICARE

We will bill Medicare for you directly from our office; assignment is accepted on all claims. Please bring your Medicare card and your secondary insurance card with you to your appointment. We will bill your secondary insurance for you only on consultation visits and surgical procedures.
